Biron Laboratory Services:

1. If you have an active International Health Insurance (IHI) plan 鈥� Medavie Blue Cross then you are eligible to benefit from direct billing for laboratory services at Biron (example: blood work).
2. To use this service, you will first need to see a doctor to obtain a blood test requisition. You may visit any doctor who has a Canadian License 鈥� 成人大片 Student Wellness Hub, Off Campus Clinics, Maple Virtual Care etc.
3. Once you have a valid blood test requisition, you may book an appointment at the closest Biron location to you. Please use the appointment booking guide below.
4. Rest assured that Biron will be responsible for transferring your blood test results back to the doctor who requested them.

Booking appointment via

1. Please enter your information as precise as you can (it is not a problem if some info is missing for the list of tests):
   
2. Enter your postal code to view nearest locations and appointment times
3. Complete all information required to book your appointment. You do not need to enter the insurance company鈥檚 name. You can also disregard 鈥楨stimated price鈥�.

What you must bring/expect at your Biron appointment:

1. You must specify to the receptionist/nurse that you are 成人大片 International Student insured with Medavie Blue Cross.
2. You must arrive with 2 pieces of identification and present them when checking in for your appointment:

3. It is mandatory to show proof of your Medavie Blue Cross card听(an electronic card on your mobile is accepted).
   
4. Your second identification can be a government-issued ID (passport, driver's license, etc.).
5. Please have your Medavie Blue Cross form completed and signed听before arriving for your appointment. You must complete the first section 鈥渕ember information鈥� and sign/date the section 鈥渕ember statement鈥�. The final section 鈥減rovider information鈥� will be completed by the Biron staff.
   Please only complete and sign either听English or French version.
6. And finally, please have your blood requisition that you received from your doctor. Please note that electronic copies will not be accepted 鈥� make sure to bring a physical copy.
